About

There shall be a fee for the ambulance operator's permit required pursuant to section 4-15 of this Code, and all permits issued under this chapter shall be valid for one year from the date of issuance. Such permits may be renewed after the termination date of the initial permit and during that same year by paying a prorated permit fee.

After Approval

If your application is approved, you will be contacted to schedule an appointment to inspect your vehicles.

Conditions and Exceptions

If application doesn’t meet all requirements it will be denied. 

If State license is revoked, the department will proceed in revoking their operating permit.

  • Copy of Medical Protocols in digital format,
  • EMS Provider is registered in Harris Count as a legal business,
  • Copies of Texas Driver License for each person listed as the owner,
  • Copies of TX DL, DSHS certification,
  • COH Driver Ambulance Permit, Copy of EMS Provider License, Certificate of Vehicle Authorization, Certificate Auto Liability Insurance.

Apply Conditions & Exceptions

Applications that are pending corrections for more than 2 weeks will be rejected and returned to the company.

Inspection What you need

Decal inspections

The company will not have a current ambulance service permit until at least one ambulance passes inspection and receives its decal.

Spot inspections

In the field ambulances are subject to spot inspections by City of Houston health officers. Refusing a spot inspection will result in enforcement action.

Renew What you need

Application process starts again. No grace period is given for expired permits or decals.
